Erin Tierney

Ernest is a dedicated family man and an experienced Specialist Support Coordinator with over 20 years of professional experience supporting individuals from diverse and complex backgrounds, including people seeking asylum, refugees, and individuals experiencing significant disadvantage. His work spans both Africa and Australia, with a strong focus on supporting people living with disability to access equitable opportunities and achieve meaningful outcomes.
He is highly skilled in delivering Specialist Support Coordination under the National Disability Insurance Scheme (NDIS), with a strong commitment to capacity building, risk mitigation, and sustainable support arrangements tailored to each participant’s needs. Previously, Ernest worked as a Local Area Coordinator (LAC) for seven years with the Disability Services Commission, now the Department of Communities, Disability Division in Western Australia. Isaiah Ajak
Isaiah holds a Bachelor of Counselling degree from the University of Notre Dame, Australia. He boasts a wealth of experience in therapeutic counselling, having dedicated his career to this noble profession since 2014. Isaiah specialises in Alcohol & Drug Counselling, as well as Family Counselling. His diverse career journey has seen him excel in various roles, including Therapeutic Community Rehabilitation, Psychosocial Support, and Mental Health Recovery. Isaiah is particularly skilled in both Individual and Group Counselling, demonstrating his versatility and dedication to helping individuals and communities.
In his role as a Community Support Coordinator and Psychosocial Recovery Coach at Konnectus Support Coordination, Isaiah can provide invaluable support to clients struggling with Mental Health (MH) and trauma-related issues, both within the hospital environment and in their homes following discharge.
Rebeka Shrestha
Rebeka is a highly experienced and dedicated Specialist Support Coordinator and Psychosocial Recovery Coach with over eight years of experience in the disability and community services sector. She works holistically to understand each person’s unique needs and aspirations, empowering them to build on their strengths and lead a more fulfilling life.
Since 2016, Renae has gained extensive experience across the NDIS and mental health sectors, including roles as a Service Coordinator and Support Coordinator. She has also worked closely with acute mental health wards, supporting participants as they transition into community living and regain independence.

With over eight years of hands-on experience in the disability sector, Emilie has developed a genuine passion for person-centred support. Starting as a frontline support worker and growing into a Senior Coordinator role, she has successfully managed three Supported Independent Living (SIL) homes. In these roles, Emilie has taken care of day-to-day operations and made sure every participant receives the personalised, compassionate care they deserve. As a House Coordinator, she’s skilled at coordinating complex care plans, working closely with multidisciplinary teams, and putting in place strategies that help people live more independently and happily.

As Referral Intake Coordinator, Dee plays a vital frontline role in welcoming new participants and guiding them through the early stages of their NDIS journey. She holds a Certificate IV in Mental Health and brings a versatile, compassionate, and skilled background in community services, disability support, and education.
With a strong foundation in person-centred practice, Dee has extensive experience supporting individuals with complex psychosocial and mental health challenges. She is highly skilled in intake triage, psychosocial assessment, and navigating complex support needs.
In addition to her intake role, Dee has also worked in service coordination, support coordination, recovery coaching, and case management. This well-rounded experience equips her with a deep understanding of the NDIS space and enables her to provide holistic support that meets people where they are in their journey.
Her empathetic and client-focused approach allows her to build rapport quickly, gather essential information with care, and match participants with the most suitable coordinator to support their goals. She ensures each referral is managed promptly, thoroughly, and with genuine compassion.

Mery Lin also teaches language and cultural classes, where she works closely with children to support both their academic and emotional development. Trisha is a Referral Intake Officer dedicated to creating a smooth and welcoming onboarding experience, helping individuals understand available services and the role of a Support Coordinator. She is in her final year of a Bachelor of Psychological Science and is working towards becoming a Behaviour Support Practitioner. With two years of experience in the NDIS industry as a support worker in both physical and mental health, Trisha is passionate about advocacy and believes everyone deserves the opportunity to be seen, heard, and understood.
Kerry Robinson
